Clinical significance of serum UMOD on prediction of acute kidney injury in patients with acute pancreatitis |

Abstract Objective To investigate the early predictive value of serum uromodulin (UMOD) on acute kidney injury (AKI) in patients with acute pancreatitis (AP). Methods A total of 193 AP inpatients in our hospital were collected from January 2016 to July 2018 in Tangshan City Workers′ Hospital and divided into AP patients with AKI (AKI group, n=31) and patients without AKI group (non-AKI group, n=162) according to the KDIGO standard. The demographic characteristics, history of chronic diseases, UMOD and indicators defined within 24 hours after admission such as APACHEⅡ score, qSOFA score and biochemical indices were compared. The correlation between AP with AKI and the serum levels of those biomarkers was investigated by multiple linear regression analysis. Receiver operating characteristic (ROC) curve was generated to determine the predictive value of serum UMOD on AKI. And area under ROC curve (AUC) of these biomarkers was compared by U-test. Results There was statistical difference of the baseline values within 24 hours after admission of serum UMOD, glomerlar filtration rate (GFR), 24 h urine volume, APACHEⅡ scores and qSOFA scores between AKI group and non-AKI group (P<0.05). Logistic regression analysis showed that the low UMOD level, APACHEⅡ scores and qSOFA scores be independent risk factors of AKI in AP patients (ORall>15, Pall <0.05). The AUC of ROC curve of UMOD on AKI within 72 h or during 72 h - 7 d were respectively 0.766 (95%CI0.631-0.908) and 0.573 (95%CI0.464-0.796). Conclusion It′s suggested that serum UMOD has some clinical value in the prediction of acute kidney injury in patients with acute pancreatitis.
