Predictive value of serum CA199 level in acute cholangitis secondary to choledocholithiasis |

Abstract Objective To investigate the early predictive value of CA199 in serum for common bile duct stones complicated with acute cholangitis. Methods A retrospective cohort study was performed to collect clinical data from 595 patients with common bile duct stones from June 2015 to June 2018. The clinical data of 595 patients with common bile duct stones from June 2015 to June 2018 were collected. Among them, 435 patients with simple bile duct stones were selected as the control group. There were 160 patients with acute cholangitis, and this part was set as the cholangitis group. Serum liver function indexes (ALT, AST) and tumor markers (CA125, AFP, CEA, CA199) were compared between the two groups. The statistically significant indicators were selected to construct the receiver operating characteristic curve (ROC). Specificity, sensitivity, and critical value of the difference indicator were calculated. Results There was no significant difference in serum liver function indexes (ALT, AST) between the two groups (P>0.05). ② The serum CA199 level was significantly higher in the cholangitis group than in the control group (P<0.05). There were no significant differences in the other four tumor markers (CA125, AFP, CEA) (P>0.05). ③TheROC curve was selected by CA199. The analysis results showed that the area under the curve was 0.797, the specificity was84.4%, the sensitivity was 76.3%, the diagnostic threshold was 49 kU/L, and the 95% confidence interval(CI)was 0.75-0.843. Conclusion The tumor marker CA199 has great clinical significance for patients with common bile duct stones who may be complicated by acute cholangitis.
