Value of IL-17 and IL-33 in evaluating the condition and prognosis of patients with acute lung injury |

Abstract Objective To investigate the correlation between serum levels of interleukin-17 (IL-17) and interleukin-33 (IL-33) in patients with acute lung injury (ALI) and APACHE Ⅱ scores in acute physiology and chronic health, and to analyze their prognostic value for ALI. Methods A total of 128 patients with ALI admitted to Haikou Third People′s Hospital from January 2017 to October 2018 were selected and divided into survival group ( n =82) and death group ( n =46) according to the survival status of ALI patients. ALI patients were divided into low risk group ( n =30), medium risk group ( n =46) and high risk group ( n =52) according to APACHE Ⅱ score. Serum IL-17 and IL-33 were detected levels by enzyme-linked immunosorbent assay. ROC curve was used to analyze the value of serum IL-17 and IL-33 levels in predicting mortality in ALI patients. Pearson correlation analysis was used to analyze the correlation between serum IL-17 and IL-33 levels and APACHE Ⅱ scores in ALI patients. Results The serum levels of IL-17 (ng/L: 27.83±7.36 vs. 14.60±4.25) and IL-33 (ng/L: 46.37±11.30 vs. 30.28±8.46) in the death group were significantly higher than those in the survival group ( P <0.05). The serum levels of IL-17 (ng/L: 26.20±7.48 vs. 18.70±6.24, 13.18 ±4.25), IL-33 (ng/L: 42.50±11.62 vs. 36.25±9.13, 27.40±7.34), APACHE Ⅱ score (29.60±3.70 vs. 18.90±3.20, 9.70±1.80) and mortality (63.5% vs. 26.1%, 6.7%) in high-risk group were significantly higher than those in low-risk group and medium-risk group ( P <0.05). ROC curve analysis showed that the best cut-off values of serum IL-17 and IL-33 levels for predicting mortality in ALI patients were 18.45 ng/L and 34.20 ng/ L, respectively. The AUC[0.890 (95% CI 0.832-0.951)] for predicting mortality of ALI patients was significantly higher than that of single item, with sensitivity and specificity of 89.4% and 84.7%. The correlation analysis showed that the serum levels of IL-17 and IL-33 in the death group were positively correlated with APACHE Ⅱ scores ( r =0.816, 0.730, P <0.01). Conclusion The changes of serum IL-17 and IL-33 levels are correlated with the severity and prognosis of ALI patients, and the combined detection of IL-17 and IL-33 is of certain value in predicting the prognosis of ALI patients.
