Study on the correlation between IMA and albumin-adjusted indexes in plasma and disease severity in patients with acute pancreatitis |

Abstract Objective To investigate the correlation between ischemia-modified albumin (IMA) and albumin-adjusted indexes (such as IMARs, adj-IMA and mean-IMA) in plasma and disease severity in patients with acute pancreatitis (AP). Methods A total of 246 AP patients hospitalized within 48 h in our hospital were selected from January 2017 to May 2018 and divided into mild AP (MAP) group (n=98), moderately severe AP (MSAP) group (n=75) and severe AP (SAP) group (n=73). Patients with MAP and MSAP were as non-SAP group (n=173). The blood samples were obtained in 24 h after hospitalization. The levels of plasma IMA, IMARs, adj-IMA and mean-IMA were detected. The correlations between IMA, IMARs, adj-IMA or mean-IMA and disease severity, bedside index of severity in acute pancreatitis (BISAP) were analyzed. Receiver operating characteristic (ROC) curve was built to analyze the predictive value of IMA and IMARs on disease severity. Results Patients in SAP group had higher IMA and IMARs levels and lower adj-IMA and mean-IMA levels than patients in non-SAP group in 24 h after hospitalization (P<0.05). Patients in SAP group had highest IMA and IMARs and lowest adj-IMA and mean-IMA values among patients in SAP, MSAP and SAP groups (P<0.05). By Pearson analysis, the IMA and IMARs values were positively related with BISAP (r=0.679, 0.725, P<0.05), and the serum adj-IMA level was negatively related with BISAP (r=-0.246, P<0.05). The AUC of ROC curve of IMA, IMARs individually or IMA combination with IMARs on prediction of SAP were 0.653 (95%CI 0.557-0.749), 0.681 (95%CI0.590-0.772) and 0.874 (95%CI0.811-0.948), respectively (P<0.05). The cut-off values of IMA, IMARs individually or IMA combination with IMARs were 79.14 U/mL, 1.78 kU/g and 2.55, respectively. Conclusion It suggests that IMA and IMARs are closely related with the severity of acute pancreatitis, which could be the possibly sensitive indexes for determining of the severity of AP patients.
