Expression of serum miR-1 and miR-499 in elderly patients with acute myocardial infarction and their relationship with left ventricular remodeling |

Abstract Objective To investigate the expression of serum microRNA-1 (miR-1) and microRNA-499 (miR-499) in elderly patients with acute myocardial infarction (AMI) and their relationship with left ventricular remodeling (LVR). Methods The 182 elderly AMI patients and the 65 control individuals admitted to our hospital from January 2015 to June 2018 were selected and divided into LVR group (n=56) and non-LVR group (n=126) according to the occurrence of LVR after AMI. Serum levels of miR-1, miR-499, cardiac troponin I (cTnI) and brain natriuretic peptide (BNP) were measured and compared. The values of serum levels of miR-1, miR-499, cTnI and BNP for LVR in elderly patients with AMI were analyzed by using the ROC curve. Pearson correlation was used to analyze the correlation between serum levels of miR-1, miR-499, cTnI and BNP and left ventricular end-diastolic volume (LVEDV). Results The serum levels of miR-1 [(5.84±1.82) vs. (3.18±1.17) vs. (0.95±0.36)], miR-499[(4.30±1.35) vs. (2.38±0.91) vs. (0.62±0.25)], cTnI[(4.18±1.63)ng/mL vs. (2.52±1.20)ng/mL vs.(0.04±0.01)ng/mL]and BNP [(918.73±265.40)ng/L vs.(265.42±30.28)ng/L vs.(38.42±3.64)ng/L] in the LVR group were significantly higher than those in the non-LVR group and the control group (P<0.05). Serum miR-1 and miR-499 predicted the area under curve (AUC) of LVR in elderly patients with AMI were 0.895 (95%CI0.831~0.962) and 0.874 (95%CI0.809~0.937). The sensitivity and specificity were 88.4%, 85.0%, 86.0% and 82.7%, respectively, when the optimal cut-off values were 4.36 and 3.15. Correlation analysis showed that serum levels of miR-1, miR-499, cTnI and BNP were positively correlated with LVEDV (r=0.847, 0.805, 0.704, 0.862, P<0.01). Conclusion The high expression of serum miR-1 and miR-499 is associated with the occurrence of LVR, and may be used as biomarkers to predict the occurrence of LVR in elderly patients with AMI.
