The diagnostic value of simplified Wells clinical score combined age-adjusted D-dimer standard in acute pulmonary embolism |

Abstract Objective To explore the diagnostic value of simplified Wells clinical score com⁃ bined age-adjusted D-dimer standard in acute pulmonary embolism. Methods A total of 206 cases of outpatients with suspected APE were collected from June 2012 to December 2015 in this retrospective analysis. All patients were performed the simplified Wells clinical score. CT pulmonary angiography was the diagnostic reference standard. D-dimer was adjusted by the patient's age. Simplified Wells clinical score and D-dimer assay,respectively and combined,were used to evaluate the probability of APE. Results Seventy-two cases were diagnosed as APE. The sensitivity of simplified Wells clinical score was 78%,with specificity of 66%,positive predictive value of 55% and negative predictive value of 85%. The sensitivity of age-adjusted standard D-dimer was 93%,with negative predictive value of 95%,while specificity and positive predictive value was only 68% and 59% respectively. The sensitivity of the com⁃ bination was 96%,specificity 92%,positive predictive value 91% and negative predictive value 96%. Conclusion The combination of simplified Wells clinical score and age- adjusted D-dimer standard plays a screening role in early diagnosis of PE,which can improve the accuracy of diagnosis and reduce the rate of misdiagnosis in Emergency Department.
