Abstract Objective To explore the correlation on myocardial injury,pulmonary infection and nail fold microcirculation after acute carbon monoxide poisoning(ACOP)and edaravone's early interven⁃ tion effects on it,to provide reference for its clinical diagnosis and treatment. Methods A total of 235 pa⁃ tients with ACOP were selected. According to the whether the patients have pulmonary infection,they were divided into group A(ACOP without pulmonary infection)with 160 cases,and group B(ACOP with pulmonary infection)with 75 cases. For cases in group A,they were divided into group A1 and A2,with 80 cases per group. Patients in group A1 were given edaravone therapy for two weeks on the basis of routinetreatment. ECG and myocardial enzymes,including aspartate aminotransferase(AST),creatine kinase (CK),creatine kinase isoenzyme MB(CK-MB),troponin I(cTnI),lactate dehydrogenase(LDH)were de⁃ tected after admission,and nail fold microcirculation(NFM)were detected by XW880 microcirculation detector,and scores of capillary loop,surrounding state,capillary loop deformity rate,and white micro-em⁃ bolus rate were evaluated by Tian-shi weighted method. Cases in group A1and A2 were given ECG,myocar⁃ dial enzymes,and NFM examination two weeks later. Correlations between myocardial enzymes and NFM indexes in group B were analyzed. Results ECG abnormalities,myocardial enzymes abnormalities in group B were both higher than that in group A after admission(χ2=12.77,7.25,P<0.01),and group B's level of myocardial enzymes and NFM index were all higher than that in group A(t=3.95~6.12,P< 0.05 or P<0.01). There were no statistical differences on ECG abnormalities,myocardial enzymes abnor⁃ malities,level of myocardial enzymes and NFM index in group A1 and A2 after admission(χ2=0.41,0.10, t=0.00~0.76,P>0.05). Two weeks later,ECG abnormalities,myocardial enzymes abnormalities,level of myocardial enzymes and NFM index in group A1and A2 were all decreased(χ2=3.89~7.45,t=4.09~8.17, P<0.05 or P<0.01),and group A1's level of myocardial enzymes and NFM index were all lower than that in group A2(t=3.67~5.33,P<0.05). Serum AST,CK,CK-MB,LDH,cTnI in group B had positive corre⁃ lation with NFM indexes,including scores of capillary loop,surrounding state,capillary loop deformity rate,and white micro-embolus rate(r=1.03~1.71,P<0.05). Conclusion The patients are liable to appear myocardial damage,lung infection after ACOP,and it has good intervention effect by treatment with edaravone. For patients with lung infection,their myocardial enzymes have positive correlation with NFM indexes,and the mechanism may be related with systematic and synchronic injury of hypoxia and oxygen free radical.
