Curative effect of recombinant human brain natriuretic peptide for septic patients with cardiac insufficiency |

Abstract Objective To investigate the curative effect of recombinant human brain natriuretic peptide(rhBNP,Xinhuosu)for septic patients with cardiac insufficiency. Methods From January 2013 to August 2015,totally 42 septic patients with cardiac insufficiency in the department of Intensive Care Unit were randomly divided into Xinhuosu groups(n=21)and control groups(n=21). Patients in control groups received anti-infection,conventional therapy for cardiac insufficiency and other symptomatic support treatment,while Xinhuosu groups received Xinhuosu additionally. The general information,cardiac function indexes and hemodynamic indexes between the two groups were respectively compared. Results Compared with before treatment,the oxygenation index(PaO2/FiO2)and urine amount were increased; the heart rate(HR),mean arterial pressure(MAP),serum creatinine,B-type brain natriuretic peptide (BNP),acute physiology study with chronic health score system Ⅱ(APACHEⅡ)scores showed differ⁃ ent degrees of decline after 24,48 and 72 h treatment,the differences was of statistical significance(all P<0.05). And Xinhuosu groups improve more significantly compared with the control groups after 48, 72 h treatment,the difference was statistically significant(all P<0.05). In addition,compared with be⁃ fore treatment,invasive hemodynamic monitoring indicators such as heart ejection fraction(GEF)in⁃ creased in different degrees,global end diastolic volume index(GEDI),peripheral vascular resistance index(SVRI)and extravascular lung water index(ELWI)of both groups were decreased after 24,48 h treatment,the difference was statistically significant(all P<0.05). And Xinhuosu groups improved more significantly compared with the control groups after 24,48 h treatment,the difference was statisti⁃ cally significant(all P<0.05). Compared with before treatment,cardiac function index(CI)of both groups were increased after 24,48 h treatment,and the differences were statistically significant 48 h af⁃ ter treatment(all P<0.05). Meanwhile,compared with the same period in two groups,the increase rate of Xinhuosu groups was higher than that in the control groups,but the difference was not statistically sig⁃ nificant(all P>0.05). Conclusion rh-BNP can improve the cardiac function in patients with sepsis and cardiac insufficiency.
