Comparison between noninvasive hemoglobin(SpHb) monitoring and venous hemoglobin measurement in emergency department patients |

Abstract Objective To explore the accuracy of noninvasive hemoglobin(SpHb) monitoring in emergency patients, and to evaluate its application value in Emergency Department. Methods 300 patients in Emergency Department of Zhujiang Hospital, Southern Medical University, from October 2017 to January 2018, were enrolled finally. Noninvasive hemoglobin monitoring by spectrophotometry was reported by NT1G, while the venous blood samples were collected and true hemoglobin level (tHb) was measured by Sysmex XE-5000 automatic hematology analyzer. The precision which SpHb compared with tHb was determined by using both Pearson correlation and Bland-Altman analysis. Results Three hundred hemoglobin measurements were collected in this study. Excellent correlation was reported between both measures with Pearson correlation coefficient of 0.862 (P<0.001), and regression equation of y=-0.239+1.071x. Bland-Altman analysis showed that the average difference between SpHb and tHb was -0.59 g/dL (bias) with a standard deviation of the mean bias of 1.09 g/dL, and the 95% CI of agreement was -2.73~1.55 g/dL. The bias between SpHb and tHb was exceeding the clinically acceptable range, which meaned that the consistency was not enough. The chi-square test of the fourfold table showed that the diagnosis of blood transfusion with SpHb was of high sensitivity, high specificity, and high diagnostic accuracy. Conclusion There is a significant correlation between SpHb and tHb in emergency department patients. SpHb measurement can be used for predicting the tHb, and monitoring the changes of hemoglobin in emergency departments. SpHb plays an important role in guiding procedures for blood transfusion decision-making. However, SpHb monitoring could not replace the venous hemoglobin measurement because of the low consistency. The accuracy of SpHb monitoring needs to be further improved.
