BISAP评分联合中性粒细胞/淋巴细胞比值对急性胰腺炎病情的早期预测价值

摘要目的研究急性胰腺炎严重程度床边指数(BISAP)评分联合中性粒细胞/淋巴细胞比值(NLR)对急性胰腺炎(AP)病情严重程度的早期预测价值。方法回顾性收集2016年11月至2018年10月青岛大学附属青岛市立医院急诊科收治的AP患者165例，按病情严重程度分为三组：轻症急性胰腺炎组(MAP组102例)、中度重症急性胰腺炎组(MSAP组33例)及重症急性胰腺炎组(SAP组30例)。患者入院24 h内抽取外周静脉血行血常规、肝肾功能等实验室检查，并行CT影像学检查，分别计算BISAP评分、NLR，比较三组数据差异有无统计学意义。同时绘制受试者工作特征曲线（ROC曲线），研究BISAP评分、NLR及BISAP评分联合NLR对病情严重程度的预测价值。结果 MAP组、MSAP组和SAP组BISAP评分（0.57±0.62，1.47±0.96，2.50±1.04）、NLR（4.36±3.47，8.30±2.10，11.54±6.63）均依次升高，三组患者BISAP评分及NLR比较，差异均有统计学意义(P<0．05)，NLR与BISAP评分呈正相关(r=0.54,P<0.01)。BISAP评分、NLR及BISAP评分联合NLR预测AP病情的ROC曲线下面积（AUC）分别为0．878（95%CI0.801～0.955，P<0.001）、0．791（95%CI0.700～0.882，P<0.001）、0.911（95%CI 0.839～0.983，P<0.001），敏感度分别为83.3 %、63.3%、83.3%，特异度分别为81.5%、83.1%、95.4%；联合诊断的AUC>BISAP评分或NLR单独诊断的AUC, 差异有统计学意义(均P<0.05)。结论 AP患者BISAP评分及NLR均随AP严重程度增加而上升，均能较好地预测SAP，两者联合对AP患者病情的早期预测价值更高。
