血糖波动对动脉瘤性蛛网膜下腔出血患者预后价值的探讨

摘要目的探讨在标准血糖控制方案下动脉瘤性蛛网膜下腔出血(aneurismal subarachnoid haemorrhage, aSAH )患者血糖波动对病情严重程度以及预后的预测价值。方法采用前瞻性、随机对照方法，选取2015-06～2017-06入住我院重症监护病房（intensive care unit, ICU）的aSAH患者65例。根据患者28 d是否存活分为存活组(38例)和死亡组(27例)。采用动态血糖监测系统（continuous glucose monitoring system, CGMS）在入院即刻对65例患者进行72 h连续动态血糖监测。比较两组患者的平均血糖（mean blood glucose, MBG）、血糖标准差（standard deviation of blood glucose, SDBG）、最大血糖波动幅度（the largest amplitude of glycemic excursions, LAGE）、日内平均血糖波动幅度（mean amplitude of glycemic excursions, MAGE）及日间血糖平均绝对差（mean of daily differences, MODD）。同时测定血乳酸（入院即刻）、入院24 h内急性生理学与慢性健康状况评分Ⅱ（APACHEⅡ评分）；随访28 d生存率，比较存活组和死亡组之间动态血糖监测指标、乳酸水平以及24 h APACHEⅡ评分对aSAH病情及预后的评估价值。结果死亡组患者MBG、SDBG、LAGE、MAGE及MODD水平均高于存活组（P均<0.05）。ROC曲线下面积提示，MAGE预测患者预后的准确性较高（曲线下面积0.819，95%CI为0.706 ～0.931，P<0.001）。二元Logistic回归分析显示，MAGE和24 h APACHEⅡ评分均是预测aSAH患者28 d病死率的独立危险因素。结论 MAGE 和24 h APACHE Ⅱ评分可作为判断aSAH患者预后的独立预测因子，且MAGE预测价值明显高于APACHEⅡ评分。
