CERTAIN电子清单查房对机械通气患者临床指导价值的研究

摘要目的评价CERTAIN电子清单查房对重症医学科（ICU）机械通气患者通气时间及呼吸机相关性肺炎（VAP）等指标的影响。方法选择2016-01～2016-12我院ICU收治的173例需机械通气的患者，采用随机数字法将患者分为常规查房组（n=86）和电子清单查房组（n=87）。常规查房组按日常模式查房，电子清单查房组每日晨间查房时需核对CHECKLIST表单，并对治疗做逐项调整。分析两组患者的临床资料，对两组镇静时间、机械通气时间、VAP的发生率及ICU住院时间等进行统计学分析。结果与常规查房组比较，电子清单查房组镇静时间［(6.32±4.27)d vs.（4.52±3.65)d］、机械通气时间［(7.79±5.16)d vs.（6.34±4.24)d］及ICU住院时间［(13.65±7.24)d vs.（10.47±6.47)d］等均明显缩短（P<0.05），VAP发生率明显降低（10.3% vs. 22.1%，P<0.05），但28 d病死率两组比较差异无统计学意义（P>0.05）。结论 CERTAIN电子清单查房可以缩短患者的镇静时间、机械通气时间和ICU住院时间，降低机械通气患者VAP发生率。
