糖皮质激素对急性呼吸窘迫综合征血管外肺水的影响

摘要目的探讨糖皮质激素对急性呼吸窘迫综合征（ARDS）患者血管外肺水（EVLWI）、肺血管通透性（PVPI）、氧合指数（PaO2/FiO2）、急性生理与慢性健康Ⅱ评分（APACHEⅡ）及预后的影响。方法分析本院重症医学科（ICU）ARDS 并机械通气患者40 例，分为治疗组与对照组（每组20 例），对照组常规治疗，治疗组在对照组治疗的基础上加用糖皮质激素。比较两组患者治疗前、治疗后24、48、72 h EVLWI、PVPI、PaO2/FiO2、APACHEⅡ评分的变化及机械通气时间、住ICU 时间。结果两组患者治疗前EVLWI、PVPI、PaO2/FiO2 、APACHEⅡ评分比较差异无统计学意义（P 均＞0.05）。治疗后EVLWI、PVPI、APACHEⅡ评分均有不同程度下降，PaO2/FiO2 升高。治疗组EVLWI 及PVPI 在治疗后24、48 及72 h，APACHEⅡ评分在治疗后48 h、72 h 均显著低于对照组（P 均＜0.05），治疗组PaO2/FiO2 在治疗后48 h、72 h 均显著高于对照组（P 均＜0.05）。治疗组机械通气时间及住ICU 时间较对照组明显缩短（P＜0.05）。结论糖皮质激素能降低ARDS 患者EVL⁃WI、PVPI，改善氧合，缩短机械通气时间和住ICU 时间。
